Medical Services

GFMC is a working to be a “Patient-Centered Medical Home” (PCMH) that coordinates your care - putting the focus of your health care on you, where it belongs.
 
We pledge to:
  •  listen to your questions and concerns and to clearly explain disease, treatment, and results
  •  coordinate your care whether with specialists or resources in the community
  • provide clear instructions about your treatment goals and future plans for every visit
 
We ask you to:
  • ask questions and actively participate in your care
  • provide your medical history, and other important information, including any changes in your health
  • sign a Transfer of Medical Records form to have your medical records released and return it to the front desk
  • inform us whenever you use any other health system such as the emergency room or a specialist
 
We offer a variety of medical services as well as Anti-Aging and Skin Care enhancement procedures.  Below are just some of the many medical procedures that we offer.  If you have any questions about medical spa services or wish to schedule an appointment for an initial consultation, please feel free to give us a call.  We welcome the opportunity to talk with you.

Geriatrics and Family Medicine of Columbus is proud to inform you that our practice now offers the opportunity to use the power of the web to track the most important aspects of your healthcare through our office.  The Patient Portal enables our patients to communicate with our doctors, nurses, and staff members easily, safely, and securely via the Internet.

Participating patients are given secure User IDs and passwords, enabling them to access the Portal to view their personal and private documents, including lab and diagnostic test results, educational information, billing statements, and other health information.

Through the Patient Portal, you are able to:

  • ask questions of doctor, nurses, and staff members
  •  request prescription refills and referrals
  •  set up appointments
  •  examine your current and past statements

… all from the comfort of your home, whenever it is convenient for you!

By using the Patient Portal, you no longer have to call the office, leave a message, and wait for a response to get the results of your lab work; those results will be available to you through the Portal.  You can also send a message to the office through the Portal and expect a prompt reply.
